Northern Lincolnshire Shared Care Guidelines

These are the Shared Care Guidelines for Northern Lincolnshire. These demonstrate how the patient transition between secondary and primary care settings will take place and how correct stabilisation, monitoring and prescribing will take place to improve the outcome for patients. There will also be guidance on the structure of the interface between primary care administrated by the North Lincolnshire and the North East Lincolnshire Clinical Commissioning Groups and secondary care administrated by the Northern Lincolnshire and Goole NHS Foundation Trust.
